تشخیص سیروز صفراوی اولیه
آنتی بادی های ضد متوکندریایی (AMA) در بیش از 90 درصد از بیماران مبتلا به سیروز صفراوی اولیه (PBC) با روش ایمونوفلورسانس غیر مستقیم قابل تشخیص هستند.همچنین این روش در دیگر بیماری ها قادر به شناسایی AMA با اختصاصیت متفاوت است.آنتی ژنهای میتوکندریایی که توسط AMA در سرم بیماران تشخیص داده می شوند ، از نظر عددی بین M1 تا M9 طبقه بندی شده اند ،با کمپلکس آنتی ژنی M2 توسط AMA در سرم های بیماران مبتلا به PBC شناسایی می گردد.آنتی ژن M2 از پروتئین های آنزیمی کمپلکس 2-oxoacid dehhydrogenase تشکیل شده است که بر روی غشاهای میتوکندری داخلی قرار دارند.در این گروه از آنتی ژنها کمپلکس پیروات دهیدروژناز و کمپلکس 2-اگزوگلوتارات دهیدروژناز موجود است.
بسته به نوع کیت و روش انجام آزمایش مقادیر مرجع ممکن است متفاوت باشند.مقادیر طبیعی برای تمام سنین به شرح زیر است :
Negative: <0.1 Units
Borderline: 0.1-0.3 Units
Weakly positive: 0.4-0.9 Units
Positive: > or =1.0 Units
Negative: <0.1 Units
Borderline: 0.1-0.3 Units
Weakly positive: 0.4-0.9 Units
Positive: > or =1.0 Units
نتایج مثبت آنتی بادی ضد میتوکندریایی (AMA) با اختصاصیت M2 برای سیروز صفراوی اولیه (PBC) بسیار اختصاصی است و نتایج منفی کاذب نادر است.
یک نتیجه مثبت برای AMA با اختصاصیت M2 در یک بیمار با تظاهرات بالینی PBC عملا برای این بیماری تشخیصی است.

سطح آنتی بادی ضد میتوکندریای (AMA) برای نشان دادن مرحله یا پیش آگهی بیماری یا برای نظارت بر روند بیماری مفید نیست.نتایج مثبت (به ندرت) در بیماران مبتلا به CREST (کلسینوز ، پدیده رینود ، کم تحرکی مری ، اسکلروداکتیلی و تلانژکتازی) ، بستگان بیماران مبتلا به سیروز صفراوی اولیه و سایر بیماریهای خود ایمنی مشاهده می شود.
